Baby Loss Certificate

The Department of Health and Social Care - DHSC's  are now extended the baby loss certificate service to cover all historic losses, no matter how long ago. From 9th October 2024 there will be no backdate which means more grieving parents can request the voluntary certificate to recognise their loss of a baby who died before 28 weeks gestation prior to 1992 and thereafter 24 weeks.

You can ask for a certificate if all the following apply::

  • Your baby died before 24 weeks of pregnancy (or 28 weeks before 1 October 1992)
  • You’re one of the baby’s parents or surrogate
  • You’re at least 16 years old
  • You live in England
